Successful iliac artery angioplasty improves the inflow andaugments collateral blood flow, thus leading to moredurable results even when other segments are affected inthe same leg. Furthermore, iliac artery endovascular therapyserves as an excellent adjunctive procedure for preservinginflow to surgical bypass grafts in patients with coexistentinfrainguinal disease. Inflow iliac artery balloon angioplastymay be performed simultaneously or staged with openinfrainguinal vascular reconstruction in patients withmultilevel occlusive diseases of the lower extremity. The76 Mousa et aladvantages to patients include one combined procedure totreat lower extremity ischemia. This approach is safe andeffective, has satisfactory early results, and should be part ofthe armamentarium of vascular surgeons.
